We are talking about the Conference on Digestive System Surgery, today on the eve of its 30th edition, organized as usual by Professor Giorgio Palazzini, in Rome, and has now become one of the is a reference event in the field of training. The participation is very high: as many as 1900 participants during the 29th edition of 2018, with 100 live connections from five continents and over 33,000 subscribers on the web of the surgery congress who followed the event streamed from more than 60 countries.

The program of the congress is really rich: alongside symposiums and roundtables, as many as 100 connected operating rooms in Italy and over 30 from abroad, with 'live surgery' interventions broadcast in HD and 4K, which clearly makes you understand the organizational effort that such an event requires.

On the eve of the 30th edition, today Professor Palazzini remembers the history of his congress: "'This congress was born in 1985 when technology allowed only small point-to-point connections from 10 Kilometers away. With the evolution of the internet we had the opportunity to connect with the five continents and so now we start with connections from Japan, Australia and slowly we come to Italy and then we close the event ending in California'" explains the Professor.

The importance of the congress has grown over time, so much so that it is supported by specialization schools in surgery and it is recognized for the professional training of surgeons.

Those who participate in the congress have the opportunity to access 2 FAD courses that give as many as 50 credits per year to surgeons and operating theatre nurses: in fact, in conjunction with the congress was held the 16th  edition of the for Course for Operating Theatre Nurses, born from the joint venture with AICO  (Operating Theatre Nurses Association)  which could soon be joined by a specific training path for anesthesiologists.

These numbers easily explain how the importance and fame of the congress dedicated to the surgery of the digestive system has now largely crossed national borders, making it an unmissable appointment for surgeons and operators for whom it has simply become 'the Palazzini': "It is the other colleagues who call it ''Palazzini - comments the Professor - for me remains the Conference on Digestive System Surgery”.

A success decreed by an organizational machine and possible thanks to an impressive task force of professionals that sees about 250 people working on the event.

In order to be able to broadcast live the 100 total links of live surgery, everything must be perfect, so until a few minutes before the opening of the event the technicians relentlessly test internet networks and connections by checking carefully every single detail, every single cable, projector and screen in the meeting room, as explained by the technical manager Gaetano Fusaro: ''In the last edition of the Palazzini’s congress we had 16 connections at the same time on 16 different screens, so we should  synchronize the entrance and the release of the surgeries. We start early in the morning from 7.30am from Australia, Japan and other intercontinental connections and then we get closer to Europe and finally to Italy”.

The images from all the connected operating rooms online arrive in the Auditorium where the team networking technicians code them and direct them to the video direction, as Explains Fabio Campo di Tim: "We have a band in Auditorium of 2 gigabit through 2 redundant optics fibers, from here we then connect the video conferencing codecs that allow to obtain video audio communication to hospitals, through this console we monitor the status of the network components of both the Auditorium and the peripheral locations”. The video audio signal from each operating room is then combined with the commentator of the operation present in the room, while the audio direction followed by the manager Fabio Caratelli, it is the task of connecting the surgeons engaged in operating room with the speakers in the auditorium: ''The job is to sort all the signals of the various connections in such a way that each commentator can speak individually with the Operating Room, in 2018 we had 16 on the air in and everyone can listen, each individually, to the operation that interests them most”.'

At this point audio signal and video images are ready to be broadcast in the room, directed by Vito Ceo, Video Editor: "First we send the video signal, then we send the audio signal back so that a moderator from the room can dialogue with those who intervene live, at that point we have this wall of 16 screens that each broadcast a different operation, and thanks to the headphones every congress participant can follow the operation of his interest, all live''.

As mentioned before, the Palazzini can also be followed on the web in live streaming whose responsibility rests with the general surgeon Gabriele La Gioia: 'Since six years you have the opportunity to follow via the internet, both from mobile and from computers, this congress, with 16 channels 33,000 subscribers who follow the live streams from the various operating theatres at the same time. The difficulty lies in synchronizing all these operating rooms, synchronizing computers on the live stream and preventing there being defects between the transmitter, the receiver and the person who sees”.

The technical manager Gaetano Fusaro, Professor Palazzini's right-hand man in the organization of the congress says: "This congress has given me a lot of satisfaction in these 29 years that I follow Prof Palazzini. There is a lot of work, but I have to thank all my technicians who are really good and professional".

The impressive work of the technicians is joined by that of the surgeons and professionals who interact directly with the operating rooms, commenting on the operations highlighting the focal steps and the exceptionality: "It is a unique opportunity for a unique exchange" explains Dr. Marco Enrico Freschi, one of the speakers: a unique opportunity to learn who is approaching this type of surgery, but also to be able to discuss and try to improve more and more the approach to pathology, the approach to technology that is now a permanent companion of the surgeon”.

"The novelty and the constant innovation of the congress that we all call “the Palazzini"   has always been - continues the colleague Ferdinando Cafiero - the ability to show extremely new, extremely innovative surgery made by very experienced and very good surgeons".

The voice of the two speakers is also joined by that of Dr. Marco Rigamonti: "This congress is unique to have also an idea of what is the level of surgery nowadays and especially of minimally invasive laparoscopic surgery in Italy. Despite the importance of the congress, the only note in chiaroscuro is that this year the biggest Companies of multinational devices do not intend to provide Educational Grant for healthcare professionals, for this reason is advisable to take advantage of promotions to sign up with discounted rates.

What about the next edition of the congress, the one in 2019, the 30th anniversary?
